INFECTIOUS DISEASE REQUISITIONS
A specially designed Infectious Disease requisition is available from our Supplies Department or by calling Client Support. Fill in the requested information as completely as possible, providing the specific source of culture, collection time, and antibiotic therapy. If a particular organism is suspected, please note this in the "Additional Tests or Comment" area on the requisition so special culture techniques may be employed if necessary. Also include any pertinent patient diagnosis information that would be helpful in performing the tests requested.
